Information we collect
When you submit our appointment request form, we collect the information you provide: your name, email address, phone number, whether you are a new or returning patient, your preferred time, and any message you include. Please do not include detailed medical history in the form — that information is best shared directly with your dentist at your visit.
Alongside your form submission we also record basic context about your visit: the page you submitted from and, if you arrived from an ad or campaign link, the campaign source (for example a Google Ads click identifier). We use this only to understand which of our advertising brings patients to us.
With your consent (see Cookies & analytics below), we also collect website usage statistics through Google Analytics and measure advertising through Google Ads.
How we use your information
- To respond to your request and book or confirm your appointment.
- To contact you about your enquiry by phone or email.
- To keep a record of appointment requests so we can follow up appropriately.
- To measure, in aggregate, how visitors find and use our website.
We do not sell your personal information, and we do not share it except with the service providers below who help us operate the website.
Where your information is stored
Appointment requests submitted through this site are stored in our records system, Airtable, which notifies our clinic by email. The website itself is hosted by Vercel, and analytics and advertising measurement are provided by Google. These providers process information on our behalf and may store it on servers located in the United States, where it may be accessible under United States law. Personal health information collected during dental visits is handled separately under our in-office privacy practices and never passes through this website.
How long we keep it
We keep appointment-request records only as long as needed to respond to you and maintain a reasonable follow-up history, and we periodically delete older records. You may request deletion of your information at any time using the contact details below.
Your choices and rights
You may ask us to access, correct, or delete the personal information you submitted through this website. To make a request, contact us at agnesychan12@gmail.com — please mark your message "Privacy request". We handle personal information in accordance with British Columbia's Personal Information Protection Act (PIPA) and Canada's Personal Information Protection and Electronic Documents Act (PIPEDA).
